With ten classes in the game, which comes out on top? It’s easy to suggest the popular Secret Agent, but if every party member played as such, there wouldn’t be as much diversity in gameplay. So, I’ve curated this Animal Hospital (Anomaly) Class Tier List, using real gameplay experience, player consensus, and research to tell you the best Classes to invest in and swap to per round.
What I mean by this is, you should choose your Class based on preference, sure, but also how many players are in your party and their Classes. The more diversity, the better your survivability. But, even with this analogy, there are still some that are far better than others.
Alright, enough yapping, let’s get into the rankings.
Animal Hospital (Anomaly) Class Tier List
This Tier is up to date as of July 1st, 2026.
S-TIER
Secret Agent
It’s only natural that the Robux Class will make S-Tier, since the DEVs want it to be worth the money. The Secret Agent combines the Doctor and Security into a gun-slinging, sanity-buffing agent. The gun offers several rounds and is pretty effective at neutralising threats; meanwhile, the bonus sanity is great for staying alive longer.
Surgeon
Combines the best of the Doctor and Paramedic, being a speedster that gains bonus sanity when healing patients. Super well-rounded and kind of overpowered throughout the entiretey of the game, from early shifts to the later game. This Class also proves effective whether you run solos or play in a party campaign.
Doctor
The better Secretary. This Class balances utility and sanity, gaining bonus Sanity and recovering Sanity when tending to patients. In a multiplayer campaign, the player with this Class would tend to most patients whilst others work reception, deal with anomalies, and fetch items.
A-TIER
Security
Starts the game with a several-use taser, which gains additional rounds with levels. At max, it also recovers a use per shift, making this a solid, well-rounded protective class to deal with aggressive anomalies. This player would progress normally and be clutch when dealing with anomalies that breach inspection.
B-TIER
Head Nurse
The better Nurse, except you need Robux to own it. This class gains additional inventory space, allowing for more carried items. In the later shifts, this comes in clutch for time-intensive care for the influx of patients and their needs. Still, in a party, this Class seems a little too niche and situational, unless you particularly struggle with inventory management.
Secretary
The better version of the Intern. Rather than a flat Sanity bonus, the Secretary gains Sanity when checking in patients, and at higher levels, this accrues to bonus sanity. If you play in a party, this Class is great for one of the players to have, so that tasks can be divided based on efficiency, with the Secretary tending to patients whilst the others track anomalies, work reception, and move items.
Psychologist
High risk, high reward. This Class doubles all Sanity effects, positive or negative, with later levels having an increased chance to negate and ignore Sanity Loss. Has the chance to end the game or save it, but is overall a very average pick.
C-TIER
Paramedic
Maybe an unpopular tier, but I don’t find speed buffs too necessary for play. Sure, during later shifts, things do get hectic, but the Paramedic offers a very niche and situational resolve.
Nurse
Gains additional inventory space. Great for management when playing solo, but if you’re precise with patients and gameplay, this isn’t the most useful perk to have. If you play in a party, this Class feels especially lacklustre as you also have the pockets of all other players in your campaign.
D-TIER
This bottom Tier is solely reserved for the Intern. The default beginner class is automatically assigned to each new player.
Intern
The default class. Each level boosts Santiy at the start of the game by 5, starting with 10 Bonus at level 1, and gaining up to 20 Bonus at level 3. Whilst Sanity is critical to the game, it’s easy enough to manage without Bonuses for the first several shifts, making this Class feel somewhat redundant. The Intern also lacks any starting utility or bonus cash.
Frequently Asked Questions
How do I earn Animal Coins
Each Shift you survive, you’ll receive a report card based on performance. Doing well will accrue more Animal Coins than if you let patients die, or take too long to heal them. The longer you survive, the more Animal Coins per shift you’ll earn as well.
Which are the best Classes for a team of 4?
If you want to stay F2P, I recommend 2x Surgeons, 1x Doctor, and 1x Security. Or, 1x Surgeon, 1x Doctor, and 2x Security.
For P2P, I recommend 1x Secret Agent, 1x Surgeon, 1x Doctor, and 1x Security for maximised variety and gameplay skill coverage.
Where do I buy Classes?
In the main lobby, look for the ambulance outside the hospital. Step into the glowing circle to open the Classes menu and purchase one from there.
How do I level up my Class?
By having your Class equipped and playing as them during the main game, you’ll passively earn EXP. Each Class can reach level 3 before maxing out.
